Job Summary:
The Nurse Care Manager (RN) is an integral part of the patient-centered health home model. This RN Case Management role works collaboratively with the patient, the patient’s family, providers, other health plan divisions, payers, and others to develop and implement a treatment plan that supports self-management of chronic conditions.
Key Responsibilities:
Participate in the initial treatment plan development for all primary care health home enrollees assigned to the caseload. Assist in developing treatment plan health care goals for individuals with co-occurring chronic diseases.
Collects thorough and objective information about the physical, psychosocial, environmental, and financial needs and resources of individual patients. Develops specific and realistic objectives, goals, and action plans tailored to the individual needs of each patient. Implements specific interventions focused on the specific goals established for the individual. Facilitates access to necessary resources to accomplish patient-centered goals.
Reports quantifiable care measures as defined for quality measurement of care management. Track required assessments and screenings and identify treatment opportunities as well as document case management activities timely and according to Clinic policies and procedure and the Case Management Society of America Standards.
Analyzes sufficient information from all relevant resources to determine effectiveness of the case management plan. Modifies components or the entirety of the case management plan dependent upon its effectiveness in reaching desired outcomes.
Advocates for the individuals health care needs with emphasis on responsible health practices, self care, and patient safety. Collaborates with patient, family, physicians, payer representatives and other members of the healthcare team.
Assist in contacting medical providers and hospitals for admissions/discharge information and then following up with the patients.
Identifies gaps and barriers in treatment options and seeks to correct with creative care planning.
Maintains professional affiliations and enhances professional growth and development through seminars, conferences, and community involvement

Requirements:
Required Qualifications:
Graduate from RN program.
Interpersonal/human relations skills.
Preferred Qualifications:
BSN
Minimum 3 years diversified direct clinical experience in nursing or related field.
Experience with electronic medical records preferred.
